About the Role: As an MDS Coordinator , you will play a critical role in ensuring the highest quality of care for our patients. Your primary responsibility will be to collect and maintain accurate and thorough data in our Minimum Data Set (MDS) software, ensuring compliance with regulatory requirements. You will also be responsible for facilitating the MDS process, conducting concurrent and comprehensive assessments, and developing and implementing individualized care plans.
